Job summary

Vista Hill Foundation seeks a SUD Case Manager to oversee 25–30 youth/wards in collaboration with Juvenile Probation and Community Adolescent Treatment Programs. You will assess needs, craft intervention plans, and coordinate referrals for ancillary services.

You will conduct urine testing, follow-up, and monitor cases enrolled in SUDS, with home, school and treatment visits to support youth and families. Strong communication with teams and accurate monthly reporting are essential.

Qualifications

• Must be a registered or certified chemical dependency counselor in California.
• One year of experience working with SUD clients.
• Bilingual English/Spanish preferred.

Responsibilities

• Provide case management for an active caseload of 25–30 youth/wards.
• Interface between Juvenile Probation Department and Community Adolescent Treatment Programs.
• Assess, develop intervention plans and referrals.
• Conduct observed urine tests; monitor cases while in SUDS.
• Conduct home, school and treatment program visits with youth, family, providers and teachers.
• Participate in team meetings; compile and report monthly progress to Probation.
• Maintain case files; enter data in PCMS; provide end-of-month stats.
