Job summary

The Village in Hartford, CT is seeking a Case Manager for the Youth Diversion Team. You will provide comprehensive case management to ensure access to community-based services and meet referral requirements.

You will liaise with police, schools, Juvenile Probation, DCF and other providers; manage case records; conduct screenings; and prepare assessments and reports. A Bachelor's degree in social work, psychology, or counseling or equivalent experience is required.

Qualifications

• At least 3 years of direct experience in SUD treatment with CM protocols.
• Bi-lingual (English/Spanish) strongly preferred.
• Excellent oral and written skills; able to communicate with children, parents, and professionals.
• Organized and able to work independently with minimal supervision.

Responsibilities

• Focuses on engaging clients from referral through the entire course of treatment.
• Assists clients in connecting with entitlement programs and insurance.
• Conducts standardized screening for health-related social needs and offers referrals.
• Provides linkage navigation to community services for clients and families.
• Maintains and updates service data for accurate reporting.
• Develops collaborative relationships with community agencies and referral sources.

JOB SUMMARY: The Case Manager will providecomprehensive case management services to clients and theirfamilies/significant others to ensure they have access tocomprehensive, community-based services, and that all requirementsfrom referral sources are met.

Youth Diversion Team

The Case Manager will assist the Case Management Supervisor in allaspects of the Youth Diversion Team Program and have aBachelor's Degree in social work, psychology, or counseling orequivalent experience. The Case Manager will maintainstrong working relationships with the Police Department, PublicSchools, Juvenile Probation, DCF and all area service providers whomay provide services to child/youth and families appearing before thecommunity YDT; will have strong interpersonal skills and the abilityto engage resistant child/youth and family; maintain separate recordon each YDT program case, assist in maintaining the forms for thecommunity YDT; obtain incident reports, medical, academic, social, andphysical histories of child/youth referred to the community YDT andprovide these materials to the community YDT members as needed; meetwith each child/youth and his or her family to determine eligibilityfor the Youth Diversion Team Program, gain parental and child/youthagreement to participate in the Youth Diversion Team Program, ensurethe signing of appropriate forms by the parent(s) and child/youth, andperform required screenings with the child/youth and prepare reportsfor the community YDT concerning those assessments, including adetermination concerning whether more comprehensive assessments arerecommended; present individual cases to the YDT; maintain minutes ofeach YDT meeting, make arrangements with community resource providersto implement the community YDT recommendations; maintain ongoingfamily and collateral contact as needed to ensure diversionexpectations are being met; write progress reports; assist in trainingof community YDT members and community agencies in the workings of theYouth Diversion Team Program; ensure criminal records identifying thechild/youth will be destroyed in accordance with Connecticut Statutesand perform all other related duties as required.

** Travel is required throughout the state of Connecticut. Ourprograms serve the following towns:

• East Windsor Preston
• Franklin Sprague
• Scotland

KEY RESPONSIBILITIES:

• Assists clients in connecting with and gaining eligibility forentitlement programs, including public and/or private insurance programs.
• Conducts standardized screening for health-related social needsand offers resource and referral services specifically tailored tothat client.
• Provides linkage and navigation assistance for clients and theirfamilies/significant others so they can access all relevantcommunity services.
• Helps clients and their families identify and access other Villageclinical services and/or other community resources.
• Maintains and updates service data to ensure accurate reportingwhen required.
• Develops and maintains collaborative relationships with othercommunity agencies, organizations, and referral sources.
• Complies with all agency policies and procedures, HIPAAregulations, Joint Commission standards and funding sourcesrequirements including complete, timely, and accurate documentationof all relevant client information as required.
• Demonstrates cultural competence necessary to serve diverse client population.
• Participates in a collaborative team approach and engages inongoing quality improvement efforts to enhance the functioning ofthe program.
• Contributes to agency visibility and development of communitylinkages by representing The Village in community groups orcommittees, when directed by Supervisor.
• Performs other duties and responsibilities as assigned by management.
