Job summary

Alternatives in Psychological Consultation (APC) seeks a CCS Youth Lead Care Coordinator to support approximately ten to twelve CCS members. A valid vehicle and auto insurance are required, with a Bachelor's Degree in Human Services or related field and at least one year of related experience.

The role emphasizes productive field visits, supervision support, ongoing training, and coordination of care to promote quality outcomes for clients and teams.

Qualifications

• Bachelor's Degree in Human Services or related field is required.
• Strong time management, organizational, and communication skills are essential.

Responsibilities

• Provides services for CCS members (approximately ten to twelve).
• Meets productivity benchmark of at least 20 hours per week documented in Avatar.
• Counsels and mentors Care Coordinators through effective communication and team collaboration.
• Conducts client visits in the community or in clients' homes.
• Assists supervisors with feedback, coaching, training, and performance reviews.
• Ensures staff complete CCS Orientation and 8 hours of annual training.
• Reviews provider documentation and supports efficient paperwork, scheduling, and tasks.
• Participates in QA/QI tasks and maintains ethical standards.
• Attends staff meetings, supervision sessions, and in-services.
• Completes at least eight hours of continuing education annually.
• Fulfills all duties outlined in the CCS Care Coordinator job description.
• Performs other duties as assigned.
