Job summary

Professional Services Group & Community Impact Programs in Milwaukee County seeks a Care Coordinator for its CCS/CLTS Dual Care Coordination program. You will manage a caseload of youth and coordinate in-home and community services to support families, using wraparound planning.

Responsibilities include intake assessments, developing strengths-based treatment plans, coordinating with schools and providers, advocating for clients in crises, and ensuring safe transport as needed.

Qualifications

• Bachelor's degree in sociology, social work, psychology or closely related field.
• Reliable transportation with a valid driver's license and automobile insurance.
• Willingness to travel within Milwaukee County.

Responsibilities

• Provide case management services to a caseload of 12-13 clients.
• Interview and evaluate clients and families during intake and assessment.
• Create and implement goal-oriented, strengths-based treatment plans.
• Facilitate team meetings with client, family, and direct service providers.
• Develop a crisis/safety plan for the client and family.
• Develop a family support team comprising formal and informal supports.
• Collaborate with schools, mental health professionals, CF workers, and court officials.
• Advocate for clients in emergencies and crisis situations.
• Safely transport clients as needed.
