Job summary

Professional Services Group (PSG) is seeking a Care Coordinator for the CCS/CLTS Dual Care Coordination program in Milwaukee County. The role supports youth and families facing severe mental health or substance use challenges, using wraparound services to develop multi-disciplinary plans that address needs and strengths.

The Care Coordinator will manage a caseload of youth in CCS/CLTS, coordinating in-home and community services.

Qualifications

• Bachelor's degree in sociology, social work, psychology, or closely related is required.
• Must have reliable daily transportation, valid driver's license, and automobile insurance.

Responsibilities

• Provide case management for a caseload of 12-13 clients.
• Interview and evaluate clients and families during intake and assessment.
• Create and implement goal-oriented, strengths-based treatment plans.
• Facilitate team meetings with client, family, and service providers.
• Develop crisis/safety plans for clients and families.
• Develop a family support team with formal and informal supports.
• Collaborate with school personnel, mental health professionals, CFSS workers, and court officials.
• Advocate for clients in emergencies and crises.
• Safely transport clients as needed.
