We are seeking compassionate and dedicated professionals to join our team as Care Coordinators in our Youth Dual Care Coordination program. This program supports children and their families in Milwaukee County who are facing challenges with severe mental health and/or substance use disorders, with co-occurring developmental and/or physical disability needs. Our programming follows the wraparound model of service delivery to identify needed services and develop multi-disciplinary treatment plans that address both the strengths and needs of the family.
As the Care Coordinator, you will manage a caseload of youth who are dually enrolled in Comprehensive Community Services (CCS) and Children's Long-Term Support (CLTS) programs. You will coordinate in-home and community-based services to support the families’ individualized needs.
LOCATION: West Allis, WI. Local travel is required to meet with clients within their home and other community spaces throughout Milwaukee County and surrounding areas, with mileage reimbursement.
SCHEDULE: Monday-Friday 8:00am-5:00pm, with occasional evenings to accommodate families’ schedules.
PAY RATE: Candidates with a Bachelor's Degree: $24.00-26.00 per hour
Candidates with a Master's Degree: $27.00-30.00 per hour
