Job summary

Professional Services Group, Inc. in Milwaukee County seeks a Care Coordinator to support youths in the CCS/CLTS Dual Care Coordination program. You will manage a caseload, coordinate in-home and community services, and collaborate with families, schools, and providers to build strengths-based treatment plans.

This full-time role involves Monday–Friday hours with occasional evenings, mileage reimbursement, and a comprehensive benefits package designed to support staff and the communities served.

Qualifications

• Bachelor's degree in sociology, social work, psychology, or closely related field.
• Reliable transportation daily, valid driver license and automobile insurance.
• Ability to work Monday–Friday 8:00am–5:00pm with occasional evenings.

Responsibilities

• Provide case management for a caseload of 12-13 clients.
• Interview and evaluate clients and families during intake and assessment.
• Create and implement goal‑oriented, strengths‑based treatment plans.
• Facilitate team meetings with client, family, and providers.
• Develop crisis/safety plans for clients and families.
• Collaborate with school personnel, mental health professionals, and Child and Family Services workers.
