Job summary

Professional Services Group is hiring a Care Coordinator in Milwaukee County for the CCS/CLTS Dual Care Coordination program. You will manage a caseload of youth and coordinate in-home and community-based services to support families’ needs.

The role follows wraparound service delivery, with Monday–Friday hours and occasional evenings. A Bachelor's degree is required; travel within Milwaukee County is expected, with mileage reimbursement.

Qualifications

• Bachelor's degree in sociology, social work, psychology, or closely related field.

Responsibilities

• Provide case management services to a caseload of 12-13 clients.
• Interview and evaluate clients and families to assess needs and strengths.
• Create and implement goal-oriented, strengths-based treatment plans.
• Facilitate team meetings with client, family, and service providers to monitor progress.
• Develop crisis/safety plan for client and family.
• Coordinate with schools, mental health professionals, and Child and Family Services workers.
• Transport clients as needed.
