Job summary

Boys & Girls Clubs of Metro Los Angeles in Los Angeles, CA, is seeking a Peaceful Connections Case Manager for the Watts/Willowbrook Club to meet with youth, set goals, and track progress to prevent violence involvement. You will also engage families, coordinate with social services, and maintain records per program guidance.

Under supervision of an MSW, you will conduct outreach, prepare reports, schedule and attend meetings, and help youth participate in positive activities while meeting

Qualifications

• Some college in social work, psychology, sociology, human services or related field.
• Bachelor’s degree in social work, psychology, sociology, human services, or related field.
• Enrolled in an accredited master’s degree program in social work preferred.
• Two years of increasingly responsible experience working with youth populations, particularly in areas with high rates of violence.
• Bilingual in English and Spanish preferred.

Responsibilities

• Attends site and in-service meetings to convey and gather information required to perform functions.
• Assists with participant outreach by preparing and distributing outreach material, making presentations, phone calls, and scheduling meetings.
• Maintains various manual and electronic documents/reports, files, and records to provide up-to-date information and historical reference per established administrative guidelines and legal requirements.
• Assists with assessing participants’ needs, situations, personal strengths, and support networks to work with youth and determine appropriate goals.
• Coordinates with appropriate organizations and services including social services, physical and mental health services, housing support, employment education, and substance use disorder treatment.
• Monitors and tracks milestones such as employment acquisition and educational progress
• Responds to inquiries from various internal and external parties to provide information, facilitate communication among parties and provide direction.
• Meets with youth and families in compliance with program requirements, researches and refers participants to community resources.
• Tracks youth participation in positive activities and provides support to engage youth in these activities.
• Responsible for all tasks related to holding case management meetings with youth and families.
