Job summary

Child & Family Service in Ewa Beach, HI is seeking a Case Manager for the Positive Pathways Youth Mentoring Program. You will conduct outreach, screening, intake, and comprehensive assessments to engage youth and families in services.

Collaborating with mentors, schools, and community partners, you’ll develop youth-driven service plans and coordinate resources to promote resilience, growth, and positive outcomes, with occasional evening/weekend hours.

Qualifications

• High school plus additional courses or specialized training; basic computer skills.
• Bachelor’s Degree preferred; human services licensing may be considered.

Responsibilities

• Provide outreach, screening, intake, and comprehensive assessments for youth and families.
• Develop individualized service plans reflecting strengths, needs, and goals.
• Coordinate services and resources to support positive outcomes.
• Deliver trauma-informed, advocacy, and care coordination services with partners.
