Job summary

Qualifications

• Bachelor’s degree in a human services field required; Master’s preferred.
• QMHP-CS status obtainable is required.
• Minimum two years experience with mentally ill youth in a community setting preferred.

Responsibilities

• Provide ongoing case management and rehabilitation services for youth (ages 3-17).
• Coordinate services and develop the recovery plan with youth and families.
• Network with schools, probation and social service agencies to facilitate services.
• Document in electronic health records and maintain productivity standards.
• Travel as needed to provide services in community locations.
