Job summary

Bluebonnet Trails Community MHMR Center is seeking a Case Manager to deliver recovery-oriented services under the Clinical Program Manager. You will conduct psychosocial assessments, develop recovery plans, and connect clients with community resources while documenting care in the EHR.

The role emphasizes direct service, crisis intervention when needed, and collaboration with a multidisciplinary team to promote recovery and independence for youth, adults, and families in Texas.

Qualifications

• Bachelor’s degree in Human Services required.
• Strong knowledge of mental health diagnoses and treatment.
• Ability to work independently and with others in a fast-paced, recovery-oriented environment.
• Valid Texas driver’s license; maintain approved status.
• If your degree is not in Human Services, at least 24 credit hours in a related field.

Responsibilities

• Conduct routine psychosocial assessments and develop Person-Centered Recovery Plans.
• Connect individuals with community resources and provide rehabilitative services; initiate crisis intervention when needed.
• Maintain thorough and accurate electronic documentation of services.
• Prioritize direct service hours, including crisis response and case management activities.
