Diamond Experience Co. is seeking a qualified Mental Health Professional/Case Manager to provide Targeted Case Management and Rehabilitative Services to youth with serious emotional disturbances and their families.

The role requires strong collaboration with clinical teams, adherence to Medicaid guidelines, and proficiency in EHR documentation. Candidates should have a Bachelor's degree in a related field and CANS/ANSA certification, with at least two years in behavioral health.

Qualifikationen

• Must have at least a Bachelor’s degree in Social Work, Mental Health Counseling, Psychology, Sociology, or related field.
• CANS/ANSA certification is required.
• Ability to pass a drug test and background check.
• Two years of experience in behavioral health or closely related field preferred.

Aufgaben

• Adhere to Medicaid/Insurance guidelines.
• Develop service/treatment plans with the clinical team focused on child and family needs and strengths.
• Provide skill-building training in home or community using evidence-based practices.
• Offer crisis/emergency services for child/family via face‑to‑face, phone, or virtual contact.
• Maintain client documentation in the EHR and write progress notes per Texas Administrative Code.
• Enter data clearly and promptly; complete daily session notes within 24 hours of session.
• Document non-billable contacts to show follow-up or non-compliance history.
• Assist in tracking outcomes and support record review processes.
• Use active listening to build rapport with clients and families; encourage and engage them in services.
• Keep family and team informed on progress toward Service Plan goals.
• Assist clients with daily living tasks to promote independent living.
