Job summary

Pecan Valley Mh-Mr Region in Parker, TX is seeking a dedicated mental health case manager to join our care team. The role requires travel, weekend and night availability, and a four-year degree in a related field.

You will perform clinical screenings, crisis interventions, and service eligibility evaluations, coordinate linked services, and monitor progress toward treatment goals. Experience in trauma-informed care and substance abuse awareness is preferred.

Qualifications

• Requires a 4-year degree in a related field.
• Prefer 3+ years of experience with mental health crisis consumers.
• Valid Texas driver’s license and auto insurance; willing to be trained in trauma-informed care.

Responsibilities

• Provide moderately complex case management services.
• Perform clinical screenings, assessments, and service eligibility evaluations.
• Coordinate and link services and monitor progress toward treatment goals.
• Deliver crisis resolution, case management, and discharge planning.
• Provide crisis follow-up and relapse prevention services as appropriate.
• Utilize electronic health records to document assessments and plans across processes.
