Job summary

Youth-and-Family-Alternatives is seeking an Early Childhood Case Manager to support continuity of care for children 0–17 by coordinating services and resources, with a focus on ages 0–5 who are at heightened risk. Qualifications

• Full CWCM certification required.
• Knowledge of trauma-informed principles and practices, including creating a safe environment and empowering individuals.

Responsibilities

• Provide case management services from intake to discharge for assigned children and families.
• Conduct weekly home visits for high-risk cases.
• Link families to community resources and evidence-based services.
• Develop and monitor service and case plans with measurable permanency goals.
• Maintain accurate electronic and paper records and timely documentation.
