CAMBA is seeking a Social Worker for the Cornerstone program at 392 Blake Avenue, Brooklyn, NY 11212. You will support young adults (18-21) with individualized service plans and connect them to health, housing, and employment resources.

The role requires LMSW/LCSW or LMHC licensure with at least two years of urban youth experience; you will lead group sessions, document progress, and advocate for clients and families.

Qualifikationen

• Must have LMSW, LCSW or LMHC licensure.
• Two years of relevant urban youth experience preferred.
• Strong written and verbal communication skills.

Aufgaben

• Maintain client confidentiality and trust; build professional relationships.
• Lead group counseling sessions and provide peer support.
• Identify community resources and make referrals to services.
• Prepare psychosocial assessments and ISP development.
• Document progress for regular reporting (weekly/monthly/quarterly/annual).
• Support families and coordinate with the Navigator and CAMBA staff.
