Job summary

CAMBA in New York City seeks a Case Manager for the Van Dyke Cornerstone program. The role supports youth ages 18–21 with intensive case management, ISP development, and connection to city services for education, employment, health and life skills.

You will work under the Program Director, coordinating with CAMBA staff and community partners to ensure timely service delivery at the Van Dyke site. The compensation reflects $45,000–$50,000 annually, with full-time benefits.

Qualifications

• High School Diploma or GED is required.
• 2 years of relevant experience.
• Experience using Microsoft Office applications and knowledge of databases.

Responsibilities

• Maintain professional relationships with vendors and clients while preserving confidentiality.
• Follow Universal Precautions and standard procedures.
• Comply with federal, state, city and CAMBA privacy policies protecting health information.
• Keep accurate files of purchase orders and related paperwork.
• Assist with purchasing and reconciliation of orders and invoices.
• Maintain contract records for purchasing and vendors.
• Enter data into the management information system and keep files up to date.
• Review purchase orders to identify program deliveries and log inventory.
• Track back orders and coordinate with vendors for updates.
• Notify program staff of order status and attach necessary paperwork.
