Job summary

Urban Street Angels, Inc. is seeking a dedicated Case Manager to work in a shelter setting with 15-30 youth clients in San Diego, CA.

This full-time, in-person role focuses on intake, goal planning, and connection to services, with weekly progress reviews and collaboration with housing, outreach, and peer support teams. The ideal candidate has at least 1 year in case management, experience with residential programs, and a willingness to perform outreach and coordination tasks.

Qualifications

• Requires undergraduate degree (preferred; required for some programs/grants).
• Minimum of 1 year as a Case Manager or similar role.
• Experience with residential programs.
• Background check via Live Scan is required.
• Reliable transportation.
• Bilingual is a plus.

Responsibilities

• Conduct intake assessments and develop road maps for participants.
• Link participants with behavioral health providers and manage documentation.
• Assist participants with housing transitions and program moves.
• Meet weekly with participants to review progress and needs.
• Oversee day-to-day activities and support program staff.
• Maintain confidentiality and professional boundaries; manage emergencies.
• Develop tools and systems to support participant success.
• Attend county and partnership meetings regularly.
• Oversee intake and progress notes; document interactions in detail.
• Perform other duties as assigned.
