San Diego Youth Services in San Diego is seeking a Case Manager for the Victim and Survivor Services program. This full-time, non-exempt role reports to the Program Manager and supports stabilization across housing, employment, education, and health.

The Case Manager will perform outreach, crisis intervention, document care, coordinate with Probation Officers, social workers, health providers and families, and may be on-call. Salary range: $22–$24/hour; generous benefits.

Qualifikationen

• Bachelor's degree in social work or related field preferred; 2–5 years' experience with high-risk youth preferred.
• Lived experience with commercial sexual exploitation or human trafficking required.
• Bi-lingual in Spanish preferred.
• Experience in crisis intervention, group facilitation, documentation, and case management preferred.
• Must possess excellent oral and written skills.
• Work well autonomously and with a multi-disciplinary team.

Aufgaben

• Provide case management services around stabilization (housing, employment, education, health) and accompany youth to meetings/appointments/court.
• Maintain accurate documentation and contract compliance of records including file reviews.
• Conduct emergency/outreach with partners; screen for eligibility and triage as needed.
• Assist in development and maintenance of group curriculum and facilitate groups.
• Coordinate care with Probation Officers, social workers, health providers, and families.
• Be on-call on a rotating schedule; provide intensive 24hr follow-up on referrals.
• Engage youth for 90 days or longer and assist linkage to long-term resources.
• Provide outreach and training to increase community awareness.
• Seek partnerships with low-income housing providers; perform other duties as assigned.
