Job summary

Urban Street Angels, Inc. in San Diego, CA seeks a U4Y Care Coordinator to manage a caseload, connect youth to services, and support progress toward housing stability. This is a full-time, in-person role.

You will collaborate with CSS, OT, Housing Navigators, and Behavioral Health teams, maintain HMIS records, and document outcomes with attention to confidentiality and trauma-informed care.

Qualifications

• Minimum one year of experience in TAY case management or homelessness services.
• Knowledge of Housing First, trauma-informed care, and motivational interviewing.

Responsibilities

• Serve as the primary point of contact for participants from intake through program exit.
• Complete intakes and enrollments in HMIS/ETO and/or other internal systems.
• Develop and oversee individualized case plans aligned with the phased model.
• Coordinate care with CSS, OT, Housing Navigators and behavioral health.
• Monitor caseload of 45–50 participants and ensure housing stability.
