Job summary

Urban Street Angels, Inc. is seeking a Care Coordinator to manage a caseload of Transitional Age Youth, ensuring engagement and housing stability. The role focuses on coordinating services, tracking progress, and supporting participants through the program’s phased model.

Responsibilities include intake, eligibility assessments, referrals to CSS/OT/housing resources, and documenting progress in HMIS/ETO. This is a full-time, in-person position in San Diego, with a schedule Monday to Friday.

Qualifications

• Bachelor's degree or equivalent work experience (required).
• Experience working with transitional age youth or homelessness preferred.
• Excellent computer skills and proficiency with Google Workspace.
• Strong interpersonal and written/verbal communication abilities.

Responsibilities

• Serve as primary contact for participants from intake to program exit.
• Complete intakes, enrollments, and initial assessments in HMIS/ETO.
• Develop and oversee individualized case plans.
• Connect participants to services (CSS, OT, Housing Navigators, Behavioral Health).
• Manage a caseload of 45–50 participants and monitor engagement across phases.
