Job summary

Urban Street Angels, Inc. in San Diego is seeking a dedicated Case Manager to serve 15-30 youth in a shelter setting.

You will develop individualized roadmaps, coordinate with behavioral health partners, and help youth navigate housing options and program transitions. The role requires at least one year in a similar position, an undergraduate degree or-equivalent, reliable transportation, and the ability to manage confidential records.

Qualifications

• Undergraduate Degree preferred (required for some programs / grants)
• Minimum of 1 year working as a Case Manager or similar role
• Experience working with residential programs
• Must submit to a background check via Live Scan (with ongoing alerts) as required by programs, funding and licensing
• Must have reliable transportation
• Knowledge of empowerment, harm-reduction, advocacy and support techniques for vulnerable populations

Responsibilities

• Conduct intake assessments and develop roadmaps for participants' goals
• Link participants with behavioral health providers and coordinate documentation
• Connect participants to housing or next steps as they exit the shelter
• Meet weekly with participants to review progress
• Oversee day-to-day activities and contribute to program decisions
• Maintain professional boundaries and respond to emergencies as needed
• Attend County and partner meetings regularly
• Document client interactions with detailed notes
