Qualifikationen

• Bachelor's or advanced degree in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, early childhood education or early childhood intervention.
• Knowledge of community resources and case management principles, objectives, standards, and methods; and program policies and procedures.
• Ability to assess client needs, coordinate client services, and communicate effectively.
• Ability to plan and teach curriculum.

Aufgaben

• Perform moderately complex rehabilitation planning and instructional work.
• Coordinate and monitor service delivery for YES Waiver participants.
• Provide community-based services in schools, residences, or community settings.
• Assist with transportation and facilitate access to Waiver and other community services.
