Job summary

Nueces Center for Mental Health and Intellectual Disabilities is seeking a Case Manager II to support individuals with intellectual and developmental disabilities in developing their PDPs and coordinating services. The role requires frequent contact with participants, families, and providers, with emphasis on documentation and compliance.

The position is full-time, on-site in Corpus Christi, TX, with a 40-hour work week and an hourly wage of $22.20.

Qualifications

• Bachelor's degree or higher from an accredited college or university.
• Preferred: degree in social, behavioral, or human service field including psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, education, or criminal justice.
• Experience with the IDD population; case management experience; customer service.
• Bilingual in English and Spanish.

Responsibilities

• Develop Person Directed Plan (PDP) and monitor progress toward life goals.
• Coordinate with private providers and community resources including Medicaid and Social Security.
• Maintain extensive documentation within required timeframes and ensure compliance with policies.
• Communicate effectively with individuals, families, and providers to support consumer outcomes.
