Nueces Center for Mental Health and Intellectual Disabilities in Corpus Christi, TX, seeks a Case Manager II to support individuals with intellectual and developmental disabilities through the Home and Community-based Services program. You will develop Person Directed Plans, monitor progress, and coordinate with families, providers, and Medicaid to ensure services meet goals.

Qualifications

• Minimum: a bachelor's or advanced degree from an accredited college or university.
• Preferred: bachelor's or advanced degree with a major in social, behavioral, or human services fields; IDD experience; case management; customer service; bilingual in English and Spanish.

Responsibilities

• Develop and monitor the Person Directed Plan (PDP) for individuals with IDD.
• Coordinate with private providers and community resources including Medicaid and Social Security.
• Maintain documentation within required timeframes and ensure service delivery meets goals.
• Manage a flexible schedule with a caseload of about 42 individuals.
• Require reliable transportation and a valid TX driver's license.
