Job summary

Nueces Center for Mental Health and Intellectual Disabilities seeks a Case Manager II to support IDD and HCS services in Corpus Christi, TX. You will develop PDPs, coordinate with providers and Medicaid, and monitor client progress toward life goals.

The role requires strong organizational skills, trauma-informed care, and the ability to work with individuals, families and teams in a dynamic community setting. 40-hour weeks, full-time, with benefits.

Qualifications

• Minimum: bachelor's degree or higher from an accredited college or university.
• Preferred: degree in social/behavioral/human service fields; IDD experience; case management experience; bilingual in English/Spanish.

Responsibilities

• Develops Person Directed Plans (PDP) with individuals to identify services and supports.
• Monitors progress toward goals and satisfaction with services under HCS and related waivers.
• Coordinates with providers, Medicaid and community resources; maintains documentation timely.
• Communicates with individuals, families and service providers; documents per program requirements.
• Caseload approximately 42 individuals; demonstrates time management and independent judgment.
