Nueces Center for Mental Health and Intellectual Disabilities is seeking a Case Manager II, ACT, MHA in Corpus Christi, TX to provide psychosocial rehabilitation, case management, and community-based support. The role emphasizes coordinating services, promoting independence, and applying individualized treatment planning.

The position requires a bachelor's or advanced degree in a social or human services field and experience in rehabilitation or counseling.

Qualifications

• Bachelor's or advanced degree in a social, behavioral, or human service field (psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, education).
• Experience in social and rehabilitative services work or in teaching and counseling persons with disabilities.
• Preferred: 1–2 years of work experience in rehabilitation services.
• Must have reliable transportation and a valid driver’s license with a good driving record.

Responsibilities

• Performs psychosocial rehabilitation planning and case management for assigned caseload.
• Coordinates consumer needs to maintain community stability and promote independence; crisis prevention and intervention.
• Delivers outpatient mental health services with individualized treatment planning under ACT supervision.
• Identifies clients' rehabilitation and service coordination needs and provides instruction in self-help and self-care.
