Qualifications

• Ability to perform strengths-based case management, including assessment, goal setting, and service planning with youth and families.
• Experience building trusting, professional relationships with diverse youth and families and navigating crisis calmly.
• Strong organizational and time-management skills to manage multiple cases and meet reporting deadlines.
• Effective verbal and written communication with youth, families, colleagues, and community partners.
• Knowledge of housing, education, mental health, and juvenile justice resources or willingness to learn quickly.
• Ability to work collaboratively in a team while exercising sound judgment and independence.
• Commitment to equity, inclusion, and culturally responsive practice.
• Proficiency with basic computer applications and case management or data entry systems.
• Bachelor’s degree in social work, psychology, human services, or related field, or equivalent combination of education and relevant experience.
• Experience in youth services, family support, or community-based work is preferred.

Responsibilities

• Conduct intake assessments and develop individualized service plans.
• Coordinate referrals to housing, mental health, education, and employment resources.
• Maintain regular contact with youth and families through meetings, calls, and outreach.
• Track progress and adjust plans as needs change.
• Collaborate with internal staff and external partners to ensure smooth service delivery.
• Complete documentation and reporting, and participate in team meetings and trainings.
• Support youth-focused programming as needed.
