Lutheran Services Carolinas is seeking a dedicated professional to support youth and families in Columbia, SC in an in-person role. You will coordinate referrals, complete documentation, and collaborate with DSS and community partners to ensure services meet youth needs.

The ideal candidate has strong communication and organizational skills, experience with assessments, and a commitment to confidentiality in a fast-paced environment. Valid driver’s license and flexibility are required.

Formación

• Strong communication and interpersonal skills, able to work independently or in a team.
• Experience with documenting in logs and progress notes with attention to confidentiality.
• Knowledge of child welfare or related services is preferred.
• Proficient in Microsoft Word and Excel; ability to manage multiple tasks.
• Flexible and adaptable in a fast-paced, field-based environment.
• Must be at least 21 years old with valid driver’s license and transportation.

Responsabilidades

• Assist referrals and placement of youth into program services.
• Complete and maintain daily logs, progress notes, and other required documentation.
• Build and maintain relationships with DSS representatives and partners.
• Attend staffing meetings and provide updates on youth progress.
• Assess youth needs and coordinate referrals for additional services.
• Transport youth to appointments and activities as needed.
• Advocate for youth and families during meetings with outside agencies.
• Maintain knowledge of DSS policies and reporting requirements.
• Apply behavior management techniques to support positive outcomes.
• Collaborate with staff, families, schools and partners.
• Complete assessments and assist with treatment recommendations.
• Perform other duties as assigned.

Bachelor's degree in a human services-related field with 2 years of experience; or Bachelor's in non-related field with 5 years; or Master's in human services-related field with 1 year
