Lutheran Services Carolinas is seeking a Care Manager to coordinate services for youth in day treatment and foster care programs in Columbia, SC. You will collaborate with youth, families, DSS representatives, and staff to support positive outcomes and continuity of care.

The role requires strong communication, organization, and documentation skills, with a focus on behavior management and youth advocacy. In-person work with occasional travel is expected in a fast-paced team setting.

Qualifications

• Bachelor's degree in a human services-related field and two years of experience working with the population served.
• Bachelor's degree in a non-related field and five years of experience working with the population served.
• Master's degree in a human services-related field and one year of child welfare experience.

Responsabilités

• Assist with referrals and placement of youth into program services.
• Complete and maintain daily logs, progress notes, and other required documentation.
• Build and maintain positive working relationships with DSS representatives and community partners.
• Attend staffing meetings and provide updates regarding youth progress and service needs.
• Assess youth needs and coordinate referrals for additional services and supports.
