Job summary

Lutheran Social Services (LSS) is hiring a Clinical-Case Manager for the New Alternatives program in Rapid City, SD. The role supports youth as they move toward adulthood, with a focus on coordinated services, case planning, and collaboration with partners.

Qualifications

• Bachelor's degree in social work or related field is required.
• Licensure as Social Worker or Social Work Associate within 6 months of employment.
• Experience with MARs preferred and record checks required.
• Must have valid auto-insurance and driver’s license.

Responsibilities

• Develop individualized case plans and conduct ongoing assessments.
• Coordinate medical, dental, and vision appointments with providers.
• Maintain accurate case documentation and community referrals.
• Transport clients to appointments and support medication management.
