Job summary

Lutheran Social Services is seeking a dedicated professional to support South Dakota DSS youth in foster care as they transition to independence. You will assess needs, provide case management and life skills training, and coordinate housing, employment, education, and healthcare supports.

You will collaborate with DSS staff, foster parents, facilities and partners to build transition plans and lead youth advocacy efforts across Western and Southwestern South Dakota.

Qualifications

• Bachelor’s degree in social work or related field required.
• Licensed Social Worker or Licensed Social Work Associate preferred.
• Must be 21+ and pass agency background checks.
• Must have active auto-insurance and a valid driver’s license.

Responsibilities

• Assess individual needs and provide comprehensive case management.
• Deliver life skills training and direct housing, employment, education, and healthcare support.
• Act as liaison with DSS staff, foster parents, facilities, and community partners.
• Develop tailored transition plans and coordinate youth advocacy activities and events.
