Bronx Family Network is seeking a full-time Case Manager to support residents in a DHS shelter in NYC. You will conduct needs assessments, develop ILPs, provide case management, crisis intervention, and connect clients with housing, health, and social services.
You will document in CARES, maintain accurate records, advocate for clients, and work closely with shelter staff to move clients toward permanent housing. Bilingual English/Spanish preferred; Microsoft Office proficiency required.

The Case Manager also plays a crucial role in connecting clients with community resources and advocating for their well‑being. As a Case Manager, you will be responsible for setting clear, measurable goals and fostering a supportive, empathetic relationship with clients based on trust and accountability. You will help clients address their needs and transition to permanent housing through direct service or referrals.
This position operates in an agency office located in a shared and open workspace. The position uses standardized office equipment such as phones, computers, fax machines, and copiers.
