Integrated Support and Facilitation in Indianapolis, IN is seeking a passionate Wraparound Facilitator to coordinate individualized care plans that foster stability, growth, and lasting change for youth and families. Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or related field.
• Experience with at-risk youth, families, or case management.
• Knowledge of wraparound processes and trauma-informed, family-centered care.
• Strong communication, active listening, and conflict resolution skills.
• Flexibility to work evenings to meet family needs.
• Valid driver's license and reliable transportation.
• Proficiency in documentation and basic computer skills.

Responsabilités

• Facilitate wraparound team meetings with families and partners to plan and implement individualized care plans.
• Build trusting, strengths-based relationships with youth and families to identify needs, goals, and natural supports.
• Coordinate services across behavioral health, education, child welfare, and community organizations.
• Monitor progress and adjust care plans as needed through ongoing assessment.
• Maintain accurate documentation, case notes, and service records in compliance with agency standards.
• Advocate for families to ensure access to appropriate services and community-based supports.
• Collaborate with a multidisciplinary team to ensure holistic, family-driven care.
