Carolina Therapeutic Services, Inc. is seeking a NC Wraparound Facilitator to engage high-need youth and families in an intensive care planning process. You will help develop, implement and monitor an HFW plan with strong family partnerships and community resources.

The role emphasizes guiding CFTs, coordinating services, maintaining documentation, and ensuring fidelity to the High-Fidelity Wraparound model. Strong collaboration and confidentiality are required.

Qualifications

• Masters Degree Applicants: degree in a related field and 1 year post-graduation experience (2,000 hours).
• Bachelors Degree Applicants: degree in a related field and 2 years post-graduation experience (4,000 hours).
• Experience with at-risk youth and families, including clinical case management or therapy.

Responsabilités

• Works with the family to integrate information from multiple sources into discovery.
• Identifies appropriate members of the Child & Family Team (CFT).
• Collects background information and plans from other agencies.
• Facilitates the CFT to develop an integrated mission to support the family.
• Convenes and facilitates CTS meetings.
• Ensures the plan aligns with family vision and crisis planning.
