Job summary

Carolina Therapeutic Services, Inc. is seeking a High-Fidelity Wraparound Facilitator in North Carolina. The role engages high-need youth and families to develop, implement, and monitor an intensive care plan with a strong family partnership.

The facilitator ensures HFW principles are delivered with fidelity, helping families build a positive future through structured activities and collaboration with the Child & Family Team.

Qualifications

• Experience should include clinical case management or practice of therapeutic interventions.
• At least two years of experience providing services for children and families.
• Experience facilitating small groups, preferably with multiple system involvement.
• Experience with case management database administration preferred.
• Must complete CTS Health initial orientation training and related certifications.

Responsibilities

• Integrate information from multiple sources into the strengths, needs and culture discovery.
• Identify appropriate members of the Child & Family Team (CFT).
• Collect background information and plans from other agencies.
• Facilitate the CFT to develop an integrated mission to support the family.
• Convenes and facilitates the CTS meetings.
• Ensure the HFW plan aligns with family vision, team mission and needs.
• Work with youth and family to implement elements of the HFW plan.
• Monitor and modify the HFW Plan with the family and CFT.
