Job summary

Pride in North Carolina, LLC is seeking a Facilitator for our High Fidelity Wraparound program to engage high-need youth and families in developing and monitoring individualized care plans. This role serves clients in the Bladen & Columbus Counties area and requires a Bachelor’s degree in human services or related field, with two years of experience preferred.

The Facilitator delivers the wraparound process with fidelity, builds lasting family relationships, and maintains accurate documentation.

Qualifications

• Required: Bachelor’s degree in human services or related field (experience may substitute).
• Two years of experience providing services for children, youth, and families.

Responsibilities

• Engage youth and families in care planning and monitoring.
• Develop integrated plans and natural support systems.
• Maintain strong relationships with families and community partners.
• Ensure fidelity to High-Fidelity Wraparound principles.
• Document all activities accurately and timely.
