Job summary

Kickapoo is seeking a Wraparound Case Worker to provide direct support to patients in Behavioral Health and New Hope. The role includes on-call after hours, transportation coordination, and collaboration with a multidisciplinary team to ensure continuity of care.

Ideal candidates have experience in behavioral health or human services; an Associate’s degree is preferred and bilingual English/Spanish is a plus. A Texas driver’s license and reliable transportation are required.

Qualifications

• Minimum qualifications: Relevant experience in behavioral health, human services, or community support.
• Preferred: Associate degree in social work, psychology, or related field; bilingual English/Spanish preferred.

Responsibilities

• Provide safe and reliable transportation for clients to appointments and services.
• Respond to after-hours calls from management regarding patient needs or incidents.
• Maintain on-call availability evenings, weekends, and holidays.
• Support patients in accessing referrals and staying engaged in their care plans.
• Document services and patient interactions accurately and on time.
• Participate in staff meetings to ensure coordination across departments.
