Job summary

Tri-County Behavioral Healthcare in Conroe, TX is seeking a Bilingual Child and Youth Wraparound Case Manager to deliver intensive, family-centered case management for children with complex behavioral health needs. The role emphasizes in-home and community visits and coordination of a care team.

The ideal candidate is bilingual, qualified as a QMHP under TAC rules, and has a Behavioral Science degree with MH experience. Mileage is reimbursed and Friday is a day off after training.

Qualifications

• Bachelor’s degree in a behavioral science field or related discipline.
• Must qualify as a QMHP under Texas TAC rules.
• Bilingual in English and Spanish required.
• 3 years MH experience preferred; strong interaction with families and agencies.
• Valid Texas driver’s license and insurability through Tri-County.

Responsibilities

• Provide wraparound, intensive case management using the NWI model with high fidelity.
• Provide clinical eligibility assessments for YES Waiver Clients.
• Billing and data entry responsibilities.
• In and out of region travel required.
• Write strength-based family stories from family input.
• Comply with Tri-County and HHSC YES Waiver policies and procedures.
