Job summary

The Kickapoo-Traditional-Tribe-of-Texas-1 is seeking a Wraparound Case Worker within the Community Services Administration. This full-time exempt role provides direct support to Behavioral Health patients, including transportation, care coordination, and on-call crisis response during and after hours.

Minimum qualifications require a 4-year degree and relevant behavioral health experience; bilingual (English/Spanish) is preferred.

Qualifications

• Relevant experience in behavioral health, human services, or community support.
• Familiarity with community resources and behavioral health support systems.
• Experience supporting individuals with behavioral health or substance use needs.
• Bilingual (English/Spanish) preferred.

Responsibilities

• Provide direct support services to patients in Behavioral Health and New Hope Department.
• Assist with transportation, coordination of care, and patient safety.
• Respond to after-hours urgent needs and participate in on-call coverage.
• Document services and maintain communication with patients and staff.
• Coordinate referrals and ensure continuity of care.
