Job summary

Kids for the Future is seeking a WRAP Care Coordinator to provide child-centered support in homes, schools, and communities. You will coordinate wraparound/family teams, develop care plans, and ensure services align with family strengths and cultural needs.

The role emphasizes collaboration with clinical and administrative staff, documentation fidelity, and maintaining confidentiality while supporting families across counties.

Qualifications

• Must have one of the listed education/experience options.
• Ability to communicate with families in a family-centered, culturally appropriate manner.
• Ability to manage multiple tasks and coordinate teams.

Responsibilities

• Schedule, coordinate and facilitate wraparound/family teams.
• Complete SNCD assessments, Care Plans and Updates, Functional Assessments / Crisis Plans / Safety Plans.
• Complete flex-fund requests when necessary.
• Provide support across counties to ensure that family and client needs are met.
• Monitor and evaluate services provided to the families.
• Facilitate or co-facilitate Family Team meetings and may type the agenda.
• Schedule and coordinate Family Team meetings and ensure supports are included.
• Contact referred families and the referral source for screening.
• Provide staff with new family team lists and approved referrals.
• Develop care plans based on strengths assessments and family needs.
• Coordinate services between agencies and maintain therapeutic relationships.
• Maintain close communication with leadership and staff to ensure high quality services.
• Attend required staff meetings and travel as needed.
• Perform other duties as assigned.
