Pressley Ridge in Ohio is seeking a Care Coordinator to deliver coordinated services to youth and families, including assessment, referrals, and reevaluation of services within OhioRISE. The role focuses on collaboration with the child and family team to develop individualized service plans and ensure timely updates.

Responsibilities include building strong relationships, coordinating across systems, attending team meetings, documenting progress, and ensuring safety nets.

Qualifications

• Requires high school diploma and valid driver’s license.
• Must have extensive travel.
• Experience in family systems, case management, child and family counseling may be required depending on education.

Responsibilities

• Provide care coordination for a caseload of 10–25 youth/families.
• Develop and maintain relationships with youth, families and key contacts.
• Assess needs and build comprehensive service plans with client and family input.
• Coordinate services across systems and facilitate Child and Family Team meetings.
• Document progress and ensure compliance with regulations and accreditation.
