Job summary

Centers for Family Development, Inc. seeks an ICCW Coordinator to provide intensive care coordination with Wraparound services for children, youth, and families with complex needs.

You will facilitate the Child and Family Team, coordinate service links, monitor outcomes, and advocate for culturally responsive, trauma-informed supports across behavioral health, education, and child welfare. The role emphasizes family-driven, youth-guided planning and collaboration with multiple systems, including

Qualifications

• Minimum bachelor’s degree required.
• Experience in family-centered, strength-based practice; comprehensive assessment; treatment planning; case management; and professional report writing.
• Knowledge ofWraparound model, System of Care philosophy, and Michigan ICCW standards preferred.

Responsibilities

• Facilitate all phases of the Wraparound process and lead Child and Family Team meetings.
• Organize, coordinate, and advocate for services across multiple child-serving systems.
• Maintain an active caseload of 8–12 families and ensure timely documentation.
