The Buckeye Ranch in Columbus, OH is seeking a Care Coordination Supervisor to lead our Care Management Entity's care coordinator teams and ensure adherence to the high fidelity wraparound model of care coordination.

You will develop training plans, supervise staff, coordinate with schools and community providers, monitor budgets, and advocate for children, youth, and families to ensure high-quality services and positive outcomes.

Qualifications

• Bachelor's degree in human services or related field; master's preferred.
• Two years of relevant working experience.
• Active LSW or LPC licensure preferred.
• Active driver's license and proof of auto insurance.

Responsibilities

• Lead care coordinator teams to ensure voice and choice in all services.
• Develop and implement training plans for care coordinators.

The Buckeye Ranch is seeking a Care Coordination Supervisor to join our Care Management Entity (CME). The Care Coordination Supervisor directs the programmatic supervision of care coordinator teams and ensures the implementation of the specific program model. The supervisor is primarily responsible for ensuring adherence to high fidelity wraparound model of care coordination, ensuring children, youth and family/caregiver centered, high quality services, positive outcomes, and satisfactory business results for the program serving children and families/caregivers of a variety of socioeconomic, racial, ethnic, and cultural backgrounds.Learn about The Buckeye Ranch:For decades, The Buckeye Ranch has been restoring hope and providing healing for children, youth, and families in Central Ohio.
