Job summary

The Buckeye Ranch in Columbus, OH is seeking a Care Management Coordinator to join their Care Management Entity. The role focuses on engaging children, youth, and families with behavioral health needs, delivering family-centered, trauma-informed, and culturally responsive care.

You will develop care plans, coordinate services, and collaborate with community partners to ensure voices of families are heard. The ideal candidate holds a degree in human services and has strong community-based

Qualifications

• Bachelor's degree in human services preferred.
• Experience with children’s behavioral health, child welfare, or related field.
• Active driver’s license and proof of auto insurance.

Responsibilities

• Complete training in High Fidelity Wraparound and ICC/MCC cases and maintain certification.
• Identify care coordination needs and support development of care plans and networks of support.
• Schedule contacts with families and providers based on needs and coordinate home, school and community services.
• Ensure services on care plans are appropriate and meet safety and treatment needs.
• Link services and refer to local community resources as needed.
• Communicate with children, youth and families in a nurturing, safe manner to support solutions.
• Develop relationships with partners and community resources for diverse populations.
• Complete all assessments, CANS, and care plan documentation.
