Job summary

Pathways Healthcare is seeking an RN to join our in-home nursing care team, delivering patient-centered rehabilitation and hospice services from our Kingston, PA office and service area. You will assess needs, plan care, and coordinate with physicians and home health staff to support independent living at home.

The role requires an active RN license in the state, WCC and/or OMS certification, and 2+ years in geriatrics. A valid driver's license and vehicle are required.

Qualifications

• Must hold an active Nursing license in the state where the position is based.
• Must hold an active Wound Care Certification (WCC) and/or Ostomy Management Specialist (OMS).
• Preferably 2+ years of nursing experience with a geriatrics focus.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status.
• Collaborate with physicians, nurse practitioners, and interdisciplinary teams to create robust plans of care.
• Address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan.
