Job summary

Pathways Healthcare is a leading home health and hospice organization providing patient-centered, in-home nursing care in Kingston and surrounding areas. Our program supports recovery at home with a focus on dignity and personalized plans of care.

We seek RNs with active state licenses and WCC certification, ideally 2+ years in geriatrics, committed to excellent bedside manner, lawful practice, and reliable transportation.

Qualifications

• Must hold an active Nursing license in the State(s) where the position is based.
• Must hold an active Wound Care Certification (WCC).
• Preferably 2+ years of nursing experience, with a focus on geriatrics.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Initiate appropriate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status and evolving needs.
• Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
• Responsively address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan development.
