Job summary

Pathways Healthcare, LLC is seeking a dedicated registered nurse to join our physician- and nurse practitioner-led home health and hospice team. You will perform in-home assessments, administer treatments, and develop plans of care for geriatric patients in a patient-centered setting.

The role emphasizes collaboration with physicians and interdisciplinary teams, ongoing professional development, and a strong commitment to compassionate, dignity-driven care in the home environment.

Qualifications

• Active RN license in the state where the position is based.
• Active Wound Care Certification (WCC).
• Preferably 2+ years of nursing experience with a focus on geriatrics.
• Nursing degree from an accredited institution.

Responsibilities

• Initiate appropriate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status and evolving needs.
• Collaborate with physicians, nurse practitioners, and interdisciplinary teams to create robust plans of care.
• Address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan development.

• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.
