Job summary

Pathways Healthcare, a physician- and nurse practitioner-led home health and hospice organization, seeks an RN to deliver high-quality in-home care in Massachusetts.

You will initiate rehabilitative nursing procedures, assess patient needs, and collaborate with a multidisciplinary team to develop robust plans of care for patients at home.

Qualifications

• Active nursing license in the state where position is based.
• Active Wound Care Certification (WCC).
• Preferably 2+ years of nursing experience, with a geriatrics focus.
• Nursing degree from an accredited institution.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial patient and family assessments to determine home care needs.
• Perform thorough physical assessments and gather medical histories.
• Develop and revise individualized care plans based on patient status.
• Collaborate with physicians, nurse practitioners, and interdisciplinary teams to create robust care plans.
• Address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and implement them in care plans.

Qualifications:

• Must hold an active Nursing license in the State where the position is based.
• Must hold an active Wound Care Certification (WCC).
• Preferably 2+ years of nursing experience, with a focus on geriatrics.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.
