Pathways Healthcare in Westwood, MA is hiring a full-time nurse to lead and deliver in-home clinical care within our Medical Rehabilitation Program. You will initiate preventive and rehabilitative nursing procedures, administer medications, and develop plans of care in collaboration with physicians and interdisciplinary teams.

The role emphasizes patient-centered care, strong bedside manners, and the ability to educate patients and caregivers.

Qualifications

• Active nursing license in Massachusetts.
• Active WOCN certification.
• 2+ years nursing experience, geriatrics preferred.
• Nursing degree from an accredited institution.
• Excellent bedside manners and communication skills.
• Knowledge of healthcare laws and regulations.
• Current BLS CPR certification.
• Valid driver’s license and insured vehicle.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather medical histories.
• Develop and revise plans of care based on patient status.
• Collaborate with physicians, nurse practitioners, and interdisciplinary teams to create Plans of Care.
• Address inquiries from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and implement them.

Responsibilitie s:

• Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
• Responsively address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan development.

Qualifications:

• Must hold an active Nursing license in the State where the position is based.
• Must hold an active Wound and Ostomy Certification (WOCN).
• Preferably 2+ years of nursing experience, with a focus on geriatrics.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

• Salary Range: $90,000 - $115,000 Annual Salary
